Schedule your Appointment

Call us at 877-888-3210 to schedule your appointment. We are open 9-5 ET, M-F. Plan to spend about four hours with us.  Alternately, you may fill out this form to request an appointment.  You'll need to provide a credit card authorization within 3 days of making your appointment.  You can download the form here and fax it back to us.

Get a Blood Test

We'll send you an e-mail containing instructions for the lab, as well as a medical questionnaire.  Take the requisition to your nearest Quest Lab. You can find the lab nearest you by clicking here. No appointment is necessary for your lab test. You must be fasting for 8-12 hours before the test is performed - nothing to eat or drink except water. Most labs open before 8AM. The results will be sent to our office 7-10 days later. Take all meds as usual on the day of the test. If you are a premenopausal female, try to have the test performed during the 17th to 21st day of your cycle.

Fill out the Questionnaire

Before your appointment, fill out a Medical Questionnaire. If you have Microsoft WORD installed, you can access a form here that you must save to your computer before filling it out. Otherwise click here for an Adobe PDF document that you can print and fill out by hand.

Be sure to send us your completed questionnaire before your visit.
